The dominant unspoken philosophical basis of medical care in the United States is a form of Cartesian reductionism that views the body as a machine and medical professionals as technicians whose job is to repair that machine. The purpose of this paper is to advocate for an alternative philosophy of medicine based on the concept of healing relationships between clinicians and patients. This is accomplished first by exploring the ethical and philosophical work of Pellegrino and Thomasma and then by connecting (...) Martin Buber's philosophical work on the nature of relationships to an empirically derived model of the medical healing relationship. The Healing Relationship Model was developed by the authors through qualitative analysis of interviews of physicians and patients. Clinician-patient healing relationships are a special form of what Buber calls I-Thou relationships, characterized by dialog and mutuality, but a mutuality limited by the inherent asymmetry of the clinician-patient relationship. The Healing Relationship Model identifies three processes necessary for such relationships to develop and be sustained: Valuing, Appreciating Power and Abiding. We explore in detail how these processes, as well as other components of the model resonate with Buber's concepts of I-Thou and I-It relationships. The resulting combined conceptual model illuminates the wholeness underlying the dual roles of clinicians as healers and providers of technical biomedicine. On the basis of our analysis, we argue that health care should be focused on healing, with I-Thou relationships at its core. (shrink)

BackgroundThe U.S. Food and Drug Administration traditionally has kept confidential significant amounts of information relevant to the approval or non-approval of specific drugs, devices, and biologics and about the regulatory status of such medical products in FDA’s pipeline.ObjectiveTo develop practical recommendations for FDA to improve its transparency to the public that FDA could implement by rulemaking or other regulatory processes without further congressional authorization. These recommendations would build on the work of FDA’s Transparency Task Force in 2010.MethodsIn 2016-2017, we convened (...) a team of academic faculty from Harvard Medical School, Brigham and Women’s Hospital, Yale Medical School, Yale Law School, and Johns Hopkins Bloomberg School of Public Health to develop recommendations through an iterative process of reviewing FDA’s practices, considering the legal and policy constraints on FDA in expanding transparency, and obtaining insights from independent observers of FDA.ResultsThe team developed 18 specific recommendations for improving FDA’s transparency to the public. Next SectionThere has been considerable debate surrounding the ethics of sham-controlled trials of procedures and interventions. Critics argue that these trials are unethical because participants assigned to the control group have no prospect of benefit from the trial, yet they are exposed to all the risks of the sham intervention. However, the placebo effect associated with sham procedures can often be substantial and has been well documented in the scientific literature. We argue that, in light of the scientific evidence supporting (...) the benefits of sham interventions for pain and Parkinson's disease that stem from the placebo effect, these sham-controlled trials should be considered as offering potential direct benefit to participants. If scientific evidence demonstrates the positive effect of placebo from sham interventions on other conditions, sham-controlled trials of interventions for the treatment of these conditions should be considered to have prospects of benefit as well. This potential benefit should be taken into account by research ethics committees in risk-benefit analyses, and be included in informed consent documents. (shrink)

By psychological benchmarks we mean categories of interaction that capture conceptually fundamental aspects of human life, specified abstractly enough to resist their identity as a mere psychological instrument, but capable of being translated into testable empirical propositions. Nine possible benchmarks are considered: autonomy, imitation, intrinsic moral value, moral accountability, privacy, reciprocity, conventionality, creativity, and authenticity of relation. Finally, we discuss how getting the right (...) group of benchmarks in human–robot interaction will, in future years, help inform on the foundational question of what constitutes essential features of being human. Literary criticism at the present moment seems ready to open its doors once again to the outside world, even if that world is only a series of other academic disciplines, each cloistered in its own way. For the reader of black African literature in French, the opening comes none too soon. The program for reading Camara Laye, Ahmadou Kourouma, and Yambo Ouologuem should never have been the program prescribed for Rousseau, Wordsworth, or Blanchot. If one is willing to read a (...) literature that might not be a rewriting of Hegel , and if the negative knowledge of recent theoretical criticism is questioned in the universality of its applications, then what is really open to a Western reader of non-Western literature? Claiming a break with his/her own culture and critical upbringing, can he/she the Other, the African, as if from an authentically African point of view, interpreting Africa in African terms, perceiving rather than projecting?The goal of breaking through the nets of Western criticism, of reading African literature in a nonethnocentric, nonprojective fashion, will remain both indisputably desirable and ultimately unattainable. No matter how many languages I learn or ethnologies I study, I cannot make myself into an African. The Western scholar’s claim to mastery of things African, albeit motivated by xenophilia rather than xenophobia, risks subjugation of the object to a new set of Western models. J. P. Makouta-M’Boukou rightly scolds Western critics who refuse to take into account the distance between themselves and African culture, and who read African literature only in function of their own cultural context.1 Wole Soyinka, more forbiddingly, complains: “We black Africans have been blandly invited to submit ourselves to a second epoch of colonisation—this time by a universal-humanoid abstraction defined and conducted by individuals whose theories and prescriptions are derived from the apprehension of their world and their history, their social neuroses and their value systems.”2 1. This report describes the rationale, approaches, organization, and resource development leading to a large-scale deletion bin map of the hexaploid wheat genome. Accompanying reports in this issue detail results from chromosome bin-mapping of expressed sequence tags representing genes onto the seven homoeologous chromosome groups and a global analysis of the entire mapped wheat EST data set. Among the resources developed were the first extensive public wheat EST collection. Described are protocols for sequencing, sequence processing, EST nomenclature, and the assembly of (...) ESTs into contigs. These contigs plus singletons were used for selection of distinct sequence motif unigenes. Selected ESTs were rearrayed, validated by 5′ and 3′ sequencing, and amplified for probing a series of wheat aneuploid and deletion stocks. Images and data for all Southern hybridizations were deposited in databases and were used by the coordinators for each of the seven homoeologous chromosome groups to validate the mapping results. Results from this project have established the foundation for future developments in wheat genomics. (shrink)

In his epistemological system Mead begins with that which the chief philosophers rejected, the novel or exceptional, and makes it central. It is central in a respect which should be carefully explained. The novel or emergent is that with reference to which a present is defined, and a present is the seat of reality. In saying this Mead does not mean that “the past” and “the future” are meaningless terms. Nor does he reduce them to a present. Rather he holds (...) that neither the past nor the future exists. In fact, they are meaningful only in relation to a present and the various pasts and futures referred to in our statement of causal conditions and predictions belong to a present, which is their seat. Mead escapes the metaphysical problem as to whether the past or the future has some sort of being even now. Certainly in so far as he has a metaphysics at all it derives from his epistemology which is primary and represents a pioneering attempt to develop a theory of knowledge answering to experimental science. Consciously or unconsciously most contemporary scientists are still under the influence of bygone epistemological and metaphysical doctrines in their explicit statements of what constitutes reality and knowing. Yet in so far as they are successful in practice obviously they are free from the blind alleys and dead ends which these older theories lead to logically. However no one, not even the philosopher, has been able to formulate a consistent set of epistemological principles from which experimental scientific procedure follows logically. In lieu of an abstract, here is a brief excerpt of the content:Notes and Discussions PARMENIDES THE PROPHET~ The latest word on Parmenides comes from a recent and exhaustive study by Leonardo Tar~n. 1 Among other illuminating and novel interpretations, Tarhn argues that Parmenides was not, after all, guilty of the confusion between the existential and copulative senses of "to be," that he did not identify thinking with Being, and that he had no conception of atemporal reality.~ In these and (...) other respects Tar~n's volume makes a valuable contribution to our understanding of Parmenides. One might judge, however, that Tars work is unusually insensitive to the religious element in Parmenides' poem, and in this respect his analysis seems to echo Reinhardt's unfortunate judgment which calls Parmenides "einen Denker, der keinen Wunsch kennt als Erkenntnis, keine Fessel ffihlt als Logik, den Gott und Geffihl gleichgiiltig lassen." s This inadequacy of TarAn's approach to Parmenides is evident in his treatment of the poem in general, and especially the prologue. The prologue to Parmenides' poem has always been a source of debate. After exploring and rejecting the allegorical interpretation of Sextus (the main source for the proem), Tar~n then systematically examines and rejects the modern interpretations which (among others) make it out to be a shamanistic rapture (Diels, Cornford), Orphic apocalypse (Burnet), religious revelation (Jaeger, Bowra), and mystical experience (Verdenius).4 TarAn judges that Parmenides did not intend his journey to be taken as a reality in any sense, largely on the grounds (1) that Parmenides speaks of his journey as a repeated experience. To take seriously the iterative expressions of the prologue (e.g. fr. 1, lines 1, 3, 8) is incompatible with interpretations which construe it to refer to a specific occasion.6 He supports the conclusion that the proem is nothing more than a literary device with the observations (2) that symbolic interpretations are incongruous with the fact that the goddess expounds her doctrine without symbols, and (3) that the appeal is to Logos, the rational, critical faculty; (4) that the goddess remains anonymous shows that she stands for no religious figure at all ("the 'revelation' is the truth discovered by Parmenides himself"); and (5) that Parmenides could hardly expect us to take seriously the reality of the goddess and other elements in the proem, the reality of which would be entirely incompatible with the thesis of the poem, viz. that reality is One and that all multiplicity and distinction is unreal. ~ z Parmenides: A Text with Translation, Commentary, and Critical Essays (Princeton, 1965). BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. BackgroundSarcomas are a diverse group of neoplasms that vary greatly in clinical presentation and responsiveness to treatment. Given the differences in the sites of involvement, rarity, and treatment modality, a multidisciplinary approach is required. Previous literature suggests patients with sarcoma suffer from poorer quality of life especially physical and functional wellbeing. Adolescent and young adult patients are an underrepresented population in cancer research and have differing factors influencing QoL.MethodsRetrospective analysis of Young Adult patients enrolled in the Sarcoma Tissue Repository at (...) University of Iowa. QoL was assessed using the self-report FACT-G questionnaire at enrollment and 12 months post-diagnosis; overall scores and the 4 wellbeing subscales were calculated. Linear mixed effects models were used to measure the association between the rate of change in FACT-G subscale scores and baseline clinical, comorbidity, and treatment characteristics.Results49 patients were identified. 57.1% of patients had a malignancy involving an extremity. Mean FACT-G scores of overall wellbeing improved from baseline to 12 months. Social and emotional wellbeing did not differ significantly between baseline and 12 months. Physical wellbeing and functional wellbeing scores improved from baseline to 12 months. No difference was seen for FACT-G overall scores for age, sex, laterality, marital status, performance status, having children, clinical stage, limb surgery, chemotherapy, or tumor size. A difference was demonstrated in physical wellbeing scores for patients with baseline limitation compared to those with no baseline limitation. A difference was demonstrated in social wellbeing based on anatomical site.ConclusionYoung adults with sarcoma treated at a tertiary center had improvements in overall reported QoL at 12 months from diagnosis. Overall baseline QoL scores on FACT-G were lower than the general adult population for YA patients with sarcoma but at 12 months became in line with general population norms. The improvements seen merit further investigation to evaluate how these change over the continuum of care. Quality of life changes may be useful outcomes of interest in sarcoma trials. (shrink)

We show that the computable dense linear order L is universal for all countable linear orders under this notion of embedding, and we establish a similar result for the computable random graph G. Such structures are said to be spectrally universal. We use our results (...) to answer a question of Goncharov, and also to characterize the possible spectra of structures as precisely the spectra of unary relations on G. Finally, we consider the extent to which all spectra of unary relations on the structure L may be realized by such embeddings, offering partial results and building the first known example of a structure whose spectrum contains precisely those degrees c with c′ ≥T 0″. Like a number of other authors, Miller uses the term “emergent” interchangeably with “unpredictable” and employs it as a property term, i.e., in contexts of the form “Event E is emergent.” As we showed in our article, however, predictability and unpredictability as well as emergence are relations; they can be predicated of an event only relatively to some body of information. Thus, a lunar eclipse is predictable by means of information including data on the locations and speeds, at some (...) particular time, of the celestial bodies involved, and the laws of celestial mechanics plus certain principles of optics; it is unpredictable on the basis of, say, alone. Consequently, the phrase “Event E is unpredictable” is incomplete, and hence meaningless, in much the same sense as the expression “Straight line l is parallel.” Similar observations apply to the term “emergent,” which is frequently used to characterize events which, in a sense examined in detail in our article, are not explainable by a micro-structure theory. Arguments from stability for liberal nationalism rely on considerations about conditions for the feasibility or stability of liberal political ideals and factual claims about the circumstances under which these conditions are fulfilled in order to argue for nationalist conclusions. Such reliance on factual claims has been criticised by among others G. A. Cohen in other contexts as ideological reifications of social reality. In order to assess whether arguments from stability within liberal nationalism, especially as formulated by David Miller, are (...) vulnerable to a comparable critique, the rationale for their reliance on factual claims is discussed on the basis of a number of concerns in John Rawls’s political liberalism. The concern with stability in liberal nationalism differs from stability in Rawls’s work, mainly because of the stronger non-ideal or ‘realist’ focus of the former. In so far as the ‘realism’ of arguments from stability for liberal nationalism is recognized, they are not vulnerable to the reification charge. But if the arguments are construed as realist, this at the same time makes for other tensions within liberal nationalism. (shrink)

If people believe that one activity is a kind of another, they also tend to believe that the second activity is a part of the first. For example, they assert that deciding is a kind of thinking and that thinking is a part of deciding. C. Fellbaum and G. A. Miller's (see record 1991-03356-001) explanation for this phenomenon is based on the idea that people interpret part of in the domain of verbs as a type of logical entailment. Their (...) explanation, however, suffers from at least 2 deficiencies. First, it fails to account for parallel effects with nouns (e.g., a contest is a kind of an activity, and an activity is a part of a contest). Second, it contains a flaw that incorrectly predicts many activities to be parts of each other (e.g., coming is part of going and going part of coming). However, a hypothesis L. J. Rips and F. G. Conrad (see record 1989-24843-001) originally proposed for the kind–part reciprocal effect avoids both of these difficulties. (shrink)

Should monkeys be used in painful and often deadly infectious disease research that may save many human lives? This is the challenging question that Anne Barnhill, Steven Joffe, and Franklin G. Miller take on in their carefully argued and compelling article “The Ethics of Infection Challenges in Primates.” The authors offer a nuanced and even-handed position that takes philosophical worries about nonhuman primate moral status seriously and still appreciates the very real value of such research for human welfare. Overall, (...) they argue for an extension and revision of the recommendations regarding chimpanzee research offered by the Institute of Medicine in 2011; the practical upshot of their argument would allow for infection challenge research for promising interventions for Ebola and Marburg virus diseases but not for smallpox or the common cold. The IOM recommendations regarding chimpanzee research put in motion an exceptionalist policy for this great ape population. Barnhill and colleagues’ proposal would enlarge the scope of that exceptionalism to embrace NHPs other than great apes. But is such exceptionalism warranted? It is not obvious to me either that the more sophisticated capacities of a species as a whole give it greater ethical protections or that less intellectually or socially sophisticated animals ought to therefore receive less protection when it comes to painful experimental interventions. Response to Emily A. Largent, Franklin G. Miller and Steven Joffe, A Prescription for Ethical Learning, Hastings Center Report, 43, s1, (S28-S29), (2013).

This is a superb introduction to the application of Chomskian linguistics in cognitive psychology and the study of linguistic performance. It is exceptionally clear, accurate, concise, and well-organized; so very well done, in fact, that it can be read by someone who knows nothing about recent linguistics or about its use in psychological experimentation, and yet can delight and profit readers who know a great deal about both subjects. Greene begins by giving a clear and discrete account of the linguistic (...) theory of Syntactic Structures and the later "standard theory" of Aspects of the Theory of Syntax. This historical approach is helpful because the first psycholinguistic applications derived from the first theory: e.g., attempts were made by George Miller, et alia to confirm that the time taken to grasp a sentence depended on the number of optional transformations involved in its generation—a "kernel sentence" with no optional transformations would take less time than its passive or negative transform, and these in turn less than, say, its passive and negative transform. Since there are neither kernel sentences nor the relevant optional transformations in Aspects standard theory, later experimentation has been concerned with confirming the psychological reality of deep, as against surface, structure: e.g., Bever’s attempt to show that, given that a subject who hears an anomalous click while listening to a sentence will psychologically displace the click to a position between major sentence constituents, there is some tendency for the displacement to operate in terms of what the grammar indicates are deep structure divisions rather than surface ones. The Times Literary Supplement of Thursday, June 19, 1969 reports that "Mr. A. V. Miller has Just produced a new version of the Science of Logic which is intended to replace that of the American translators Johnston and Struthers issued in 1929. It is clearly a most painstaking piece of work, and. it certainly succeeds in presenting Hegel in an idiom which is intelligible, if not exactly elegant..." The work contains a foreword by J. N. Findlay of Yale University, (...) and was recently published by Allen & Unwin in Britain. It is available in this country through the Humanities Press, New York. The extensive T. L. : The debate between proponents of ideal and non-ideal approaches to political philosophy has thus far been framed as a meta-level debate about normative theory. The argument of this essay will be that the ideal/non-ideal debate can be helpfully reframed as a ground-level debate within normative theory. Specifically, it can be understood as a debate within the applied normative field of professional ethics, with the profession being examined that of political philosophy itself. If the community of academic political theorists and (...) philosophers cannot help us navigate the problems we face in actual political life, they have not lived up to the moral demands of their vocation. A moderate form of what David Estlund decries as “utopophobia” is therefore an integral element of a proper professional ethic for political philosophers. The moderate utopophobe maintains that while devoting scarce time and resources to constructing utopias may sometimes be justifiable, it is never self-justifying. Utopianism is defensible only insofar as it can reasonably be expected to help inform or improve nonutopian political thinking. (shrink)

In our 2018 paper, “Hypocrisy and the Standing to Blame,” we offer an argument justifying the Nonhypocrisy Condition on the standing to blame. Benjamin Rossi (2018) has recently offered several criticisms of this view. We defend our account from Rossi’s criticisms and emphasize our account’s unique advantage: explaining why hypocritical blamers lack the standing to blame.

Floridi and Sanders, seminal work, “On the morality of artificial agents” has catalyzed attention around the moral status of computer systems that perform tasks for humans, effectively acting as “artificial agents.” Floridi and Sanders argue that the class of entities considered moral agents can be expanded to include computers if we adopt the appropriate level of abstraction. In this paper we argue that the move to distinguish levels of abstraction is far from decisive on this issue. We also argue that (...) adopting certain levels of abstraction out of context can be dangerous when the level of abstraction obscures the humans who constitute computer systems. We arrive at this critique of Floridi and Sanders by examining the debate over the moral status of computer systems using the notion of interpretive flexibility. We frame the debate as a struggle over the meaning and significance of computer systems that behave independently, and not as a debate about the ‘true’ status of autonomous systems. Our analysis leads to the conclusion that while levels of abstraction are useful for particular purposes, when it comes to agency and responsibility, computer systems should be conceptualized and identified in ways that keep them tethered to the humans who create and deploy them.
